Premature ejaculation & erectile dysfunction, semen leak

Hello Doctor,
I'm 27 years old. 185cm(6.1ft) height, 86kg weight. I used to masturbation daily for last 13 years (from my age 14yrs onwards). I controlling the masturbation for last two months I masturbate 2 times in a week. didn't get stronger erection and after insert the vagina ejaculate within 2 to 3 strokes, semen leaking when I softly touch the penis.
I'm working out in gym and I haven't taken any supplyments, I eat grilled chicken half 4 times in a week for protein without skin. Drink 3 to 4 cup of coffee everyday.
I have hair fall problem, I can see veins on my penis. I need your guidance doctor and thanks in advance

Hello Dr I'm following your medicine and I've noticed some good changes, semen doesn't leak if i softly touch my penis.
Semen comes mixed with a urine. Is acid phos 30 will stop the semen have leaks with urine??

Dr.mahfooz,after using acid phos 30 I didn't get any improvement. I have dizziness after intake the acid phos 30, can you presribe me some other medicines
