Fever from last two months

from last two months i m getting fever again and again it comes and goes and it comes again i still feel chills after its gone and there is no appetite
also feel headache or heavy head.. mostly on the back of the head and neck ..
And there also a Lump on the joint of the wrist i have posted the pic you can see it clearly it pains a little when i press it otherwise their is no pain.
