Azoospermia Treatment

Semen Analysis in Nov 2009: Azoospermia diagnosed. Repeat Semen Analysis in Dec 2009: Azoospermia confirmed.
All hormones were in normal range.
Testosterone 14.34 nmol (reference range 9.02-34.7)
FSH 3.28 mIU/ml (reference range 1.5-12.4)
LH 5.58 mIU/ml (reference range 1.7-8.6)

Ultra sound scrotum (conducted in 3 different labs): Report 1: Left sided mild varicocele with no reflux in the veins-grade 1 varicocele. Report 2: Sepiginous anechoic areas are seen in left scrotum showing color filling on Doppler. There is size variation of venous plexus after valsalva maneuver. Before 3.1 mm and after 3.7 mm. Report 3: Multiple dilated venous channels noted in scrotal sac; largest measuring 2.8 mm on valsulva maneuver.
Testicular biopsy in Mar 2010: Germ cell aplasia. No spermatogenesis seen.
Genetic test report in Jun 2010 is below:
1- Cytogenetic investigations:
Chromosomal analysis of his peripheral blood using G-T-G banding technique revealed : 46, XY Normal Karyotype

2- Mutation Screening Report

Methods: (1) Screening for microdelition in the Y chromosome AZF region
(2) Amplification of SRY gene sequence

Sample Type: DNA

Technique: Multiplex PCR using a set of 20 STSs to detect Y-chromosome micro deletions at the AZF region and the presence or absence of the SRY gene sequence.

Result:
(1) No microdelition at the AZF region of the Y chromosome
(2) SRY gene sequence is positively detected

Comments:
The possibility of small microdelition outside the area covered by the aforementioned primer sets and/or the possibility of point mutation (s) in the tested regions could not be excluded. Mosaicism could not be confirmed or excluded using this technique.

TESE conducted in Feb 2011. Around 20 samples were taken from both testis and 5 immature sperms were found. Diagnosis: “Severe Hypospermatogenesis”.
